Arms on title page painted over original shield: per fess, 1: vert a helment argent with a knot gules; 2: azure three bends or; over the fesse line a divise argen; on a chief azure three fleurs-de-lis or between the form points of a label gules; no. 139 in an old collection; old shelf marks B2, 100 and 25; purchased by J. Pierpont Morgan from Gruel in 1909; J.P. Morgan (1867-1943).

Ms. written and illuminated in Milan, Italy (calendar for use of Ravenna), in or just before 1386 (table of lunar and solar computations begins with the year 1386).
Artist: a follower of Giovannino de' Grassi.
