Red velvet over boards; on the upper cover have been applied 14th-century gilt plaques decorated in stamped relief; four walrus-ivory plaques depicting the Four Evangelists at the corners that have been dated to the late 12th century by Goldschmidt; mounted in bloodstones and crystal; at center a Crucifixion in walrus ivory with the Virgin holding a book and St. John writing in a codex, dated by Goldschmidt to ca. 1100.

Said to have been owned by the collegiate church of the Holy Apostles in Cologne; purchased by Sir Thomas Phillipps from Payne and Foss, ca. 1826; Phillipps Collection, no. 3007; purchased by J.P. Morgan (1867-1943) from the Phillipps estate in 1920.
